The Subordinate Courts were renamed “State Courts” on 7 March 2014. Unless otherwise stated, the processes and procedures remain unchanged. Where reference is made to “Subordinate Courts” in any subsidiary legislation, contract, deed or document, it shall be read as a reference to the “State Courts”, unless the context states otherwise.

These forms only apply to
claims that were filed
before 10 July 2017 (Manually).
For claims that were filed
on or after 10 July 2017 (CJTS), please log in to CJTS to file the relevant e-services.
1. Where there is a PDF format (print version) given with an MS Word format (editable version), please note that the finished document must conform with the required format as shown in the PDF version.
2. If any of the forms for proceedings at the Small Claims Tribunals are handwritten, please:
a. print and use only the PDF versions; and
b. ensure that form(s) are completed neatly and legibly. Failure to do so may result in the rejection of the form(s).

Please note that when the completed Claim Form is printed out, it should be exactly 2 pages and double-sided; otherwise the said form may not be accepted for processing/registration.
Please ascertain if your printed form conforms with the required format by referring to the PDF version of the said form.
