These forms only apply to claims that were filed before 10 July 2017 (Manually).

For claims that were filed on or after 10 July 2017 (CJTS), please log in to CJTS to file the relevant e-services.

1. Where there is a PDF format (print version) given with an MS Word format (editable version), please note that the finished document must conform with the required format as shown in the PDF version.

2. If any of the forms for proceedings at the Small Claims Tribunals are handwritten, please:

a. print and use only the PDF versions; and

b. ensure that form(s) are completed neatly and legibly. Failure to do so may result in the rejection of the form(s).
